Summary of Essential Duties:
- Serve as a clinical co-pilot, considering various needs of each patient, including medical, psycho-social, financial, and practical, to map out the best care trajectory.
- Coordinate the care and service of patients from admission to discharge.
- Assess, plan, implement, monitor, and evaluate options and services to meet individual health needs for a safe and timely discharge to the next appropriate level of care.

Educational Requirements:
- BSN preferred. Associates Degree in Nursing required. CA License required.

Licenses:
- Case Management certification and experience preferred.
- Expectation to acquire Case Management certification within 2 years of hire.
- BLS certification required.

Experience:
- Minimum 3 years of experience in Acute Care Nursing.

Physical Demands:
- Standing and walking.
