We are currently seeking a Care Coordinator who is eager to expand their experience in our Magnet-designated, teaching environment. As a Care Coordinator, you will have the opportunity to use your clinical expertise to provide the best possible care for our patients. This includes addressing their medical, psycho-social, financial, and practical needs. You will help coordinate the care and services for our patients from admission to discharge, ensuring a safe and timely transition to the appropriate level of care.

Key Responsibilities:
- Serve as a clinical co-pilot, charting the best trajectory for each patient's care by accommodating various needs.
- Coordinate the care and services for our patient population from admission through discharge.
- Assess, plan, implement, monitor, and evaluate options and services to meet individual health needs and facilitate a safe and timely discharge.

Qualifications:
- BSN preferred. Associates Degree in Nursing required, CA License required.
- Experience and certification in Case Management preferred.
- Case Management certification expected within 2 years of hire.
- BLS (Basic Life Support) certified.
- Minimum 3 years of experience in Acute Care Nursing.
- Physical demands include standing and walking.
