We would be happy to hear from you.As a Clinical Nurse II, you will assume responsibility and accountability for coordinating the treatment of patients with diverse diagnoses including liver cancer, metastatic cancer, vascular disease, vascular malformation, osteoporotic compression fracture, bone lesions in coordination with the Nurse Practitioner.Summary of Essential Duties:Educate patient/family regarding diseases, treatment, side effects and symptom managementConnect patients with resources such as home care and physical therapyMaintain communication and coordinate interdisciplinary care amongst all providers to facilitate the best care and support for each individual patient situationRoom clinic patients including vital signs, review medication list, allergies. Virtual rooming includes reviewing medications and allergiesAssess patients as they come to the exam area and facilitate communication with the MDEnsure outside records for new consultations/patients are obtained prior to the visit, including clinic formsMake post-procedure phone calls to patientsCoordinate all pre-operative clearance for patients undergoing a procedure and ensure documents are acquired in a timely manner and reviewedTriage phone calls from patients to assess immediate needs and provide solutions, contact other practitioners to facilitate solutions, and/or provide advice to the patient for appropriate measuresAssist physicians with office-based proceduresPOC testing, central port access, and insertion of peripheral IV'sQualificationsEducational Requirements:Bachelor's of Science in Nursing (state accredited program) : preferredLicenses:California State RN LicenseBCLS CertificationOther Competencies, as required byyour specialty unit (ex.
