About the role:You will be comfortable driving around Adelaide visiting elderly customers in their own homes, providing practical advice and solutions so they can maintain their independence and continue to do the activities they love. You get to chose the 2-3 days per week you'd like to work but they must be consistent.You will provide a wide variety of services, from ADL assessments and equipment prescription to home modifications and intervention services. You’ll be focused on supporting the physical, functional and cognitive abilities of our customers, with the aim of increasing mobility and reducing falls risks. Your caseload will be varied and you will play a key role in the health and wellbeing of Customers with a range of health needs, including chronic health conditions such as cardiovascular disease, musculoskeletal disorders, diabetes and dementia. You will be responsible for supporting care plans and provide solutions tailored to each customer’s individual needs.You will be the only OT in our multidisciplinary team so you'll be comfortable working on your own. Our office is based in Wayville but you will be able to work from home when not visiting customers.
